NIH Application Certification Form

Effective May 2006, the signature of the Principal Investigator is no longer required on NIH applications. Instead, NIH is requiring the University to secure and retain a written assurance from the Principal Investigator prior to submitting the application. When multiple Principal Investigators are proposed in an application, this assurance must be obtained for all named PIs.

OSP has developed a single NIH Certification Form to cover all of the certifications required by the NIH, including the assurance that all PIs involved in a multiple PI application agree to the leadership plan and subsequent progress reports.

The form should be completed, signed, and included with all NIH applications and Progress Reports submitted to OSP for review. Forms are required for all PIs, including PIs from other institutions, if the application involves multiple PIs. For multiple PI applications a Contact PI must be designated and his/her name should be included on the form.
